Abstract:

In addition to clinical and histopathologic prognostic factors of colorectal cancer, biological factors, which include markers of tumor burden, DNA ploidy levels and molecular (genetic) markers, provide very useful information regarding predictive survival, recurrence after surgery, mortality risk and response to adjuvant treatment following diagnosis of these neoplasia. These factors can explain, because of molecular and genetic heterogeneity of colorectal cancer, the marked variability in outcomes that exist in each TNM stage, depends in many cases on tumor genotypes which dictate their behavior and not just on common histopathologic criteria only. The purpose of the second section of this article is to present an overview about the variability of biological prognostic factors in colorectal cancer, discovered by modern molecular biology techniques, and to show the significant role of these factors in predicting not only survival but early recurrence and dissemination risk or, in some cases, the resistance to chemotherapy. The most important component of these factors remains molecular markers of tumors. It is estimated that in the near future, once the molecular biology of colorectal cancer will be fully elucidated, the study of molecular profile of these tumors will dictate next therapeutic decisions.
